The following information will be needed to register your child(ren):

• Certified Birth Certificate
• Immunization Record
• 2 documents showing Proof of Residency
• Custody Papers (if applicable and they must be signed by the Judge)
• Click the Registration packet and you will be directed to our Final Forms to register.
